New Patient Registration
We welcome new patients to the practice.
To register, please collect a registration form from the plastic drawers located by the surgery side entrance. Forms can be completed at home and returned to the practice along with the required identification.
For identification purposes, we ask for:
- One form of photographic ID (e.g. passport or driving licence)
- One proof of address (e.g. utility bill or bank statement)
If you are unable to provide identification, please speak to a member of our reception team, who will be happy to advise.
Patients registering from abroad on a permanent basis may be asked to provide appropriate documentation, such as a passport, visa, or work permit.
Areas Covered
We provide services to patients across the following areas: Mold, Gwernymynydd, Cadole, Maeshafn, Gwernaffield, Pantymwyn, Cilcain, Rhydymwyn, Sychdyn, Nercwys, Bryn-y-Baal, and Mynydd Isa (from Pren Hill towards Mold), as well as Northop up to the A55.
