Please attend the reception desk with your medical card, if possible. You will require your NHS number if you have been registered previously with the NHS as well as your passport or other ID and proof of address e.g. mortgage or rent agreement or a utility bill. Patients coming from abroad and wishing to register permanently for the first time will need to provide appropriate documentation eg work permit, passport, entry visa etc. You will be offered a new patient medical with a health care assistant who will check your blood pressure, height, weight etc.
Please bring a urine sample to this consultation. If any patient changes their name or address they should notify their doctor's receptionist in order that we can change our records. You will be registered with the practice as a whole rather than with a specific GP but you can ask to see a specific GP and we will do our best to accommodate you although it may not always be possible.
